    Langkah Kedua

    There are four types of letters that students can request:

    1. Certificate of Active Student Status
    2. Study Leave
    3. Withdrawal from the Program
    4. Replacement Student ID Card

    Student Leave Application

    Steps
    Step One
    • Please select the “Leave” menu.
    • Second Step
    • Next, choose the semester period for the leave application.
    • Select the number of leave semesters (maximum 2 consecutive semesters).
    • Choose the reason for leave, such as:
      • Working
      • Illness
      • Pregnancy or Childbirth
      • Others (provide your reason)
    • After that, click the “Submit Leave” button.
    Third Step
    • After clicking the “Submit Leave” button, the leave application form will appear.
    • Download this form and fill it out according to the procedure (including remaining study period).
    • If the address is incorrect, report it to BAPKM before submitting the form to:
      • Academic Advisor
      • Department Head/Program Chair
      • Dean
    • The form must be signed by:
      • The Student Applicant
      • Academic Advisor (if the name is the same as the Dean, leave it blank)
      • Department Head/Program Chair
    • Send the signed form to the email: sekretariat.mmt@gmail.com

    Student Program Withdrawal Application

    Steps
    First Step
    • Please select the menu “Resignation”.
    • Step Two
    • Next, choose the semester period for your resignation request.
    • Write the reason for resignation, for example: employment, transferring to another university, etc.
    • Click the “Submit Resignation” button after making your selections.
    Step Three
    • After clicking the “Submit Resignation” button, a page like the one above will appear. Click “Print”.
    • Step Three
    • The form above will appear. Download it.
    • Once downloaded, the form must be signed by:
      • Requesting Student (with a 10,000 IDR stamp)
      • Academic Advisor
      • Head of Department/Program
    • Send the signed form to the email: sekretariat.mmt@gmail.com
      Alternative Step
    • Please create a resignation letter using a “free template”.
    • Include your personal information such as:
      • Applicant Student Name
      • Student ID (NRP)
      • Study Program
    • Write your reason for resignation, for example: working, transferring to another university, etc.
    • Send the letter with a 10,000 IDR stamp and signed to the email: sekretariat.mmt@gmail.com
