  5. SCAN ORIGINAL DIPLOMA AND TRANSCRIPT OF BACHELOR’S, MASTER’S, DOCTORATE, PROFESSIONAL, AND SPECIALIST DEGREES. FOR FOREIGN DEGREES, INCLUDE DIPLOMA EQUIVALENCY.
