- Transkrip nilai S1 sampai pendidikan terakhir
- SK Penyetaraan Ijazah bagi lulusan luar negeri
- Surat Keterangan Sehat Jasmani dari rumah sakit status serendah-rendahnya tipe C
- Surat Keterangan Sehat Rohani dari rumah sakit dengan status serendah-rendahnya tipe C
- Surat keterangan Bebas Narkotika dari rumah sakit
- SK Beban Mengajar ditandatangani Dekan atau Surat Tugas Mengajar ditandatangani Kepala Departemen
- Surat keterangan pelaksanaan Tridharma dari Dekan
- Surat pernyataan dari Dekan yang menyatakan bahwa informasi yang ada dalam dokumen yang diusulkan adalah benar
